Community-based, free and confidential HIV/STI testing

By accessing our testing service you declare and confirm that your decision to get tested is entirely voluntary and that information given to us during pre-test or post-test counselling is accurate. Please fill out this questionnaire which will help our counsellor to perform the counselling.
Privacy and data protection
Legebitra is dedicated to the protection of your data. We treat your personal data with great care and in compliance with current data protection legislation in force in the Republic of Slovenia, above all with the Personal Data Protection Act and the European General Data Protection Regulation. Processing of data is performed by authorized persons only.
Purpose of data processing
Legebitra collects your data for the following purposes: to use them during pre- and post-test counselling; for statistical purposes in aggregated form (used in preparation of reports for the Ministry of Health and Municipality of Ljubljana); research purposes (reporting to European network of community-based testing points – Cobatest Network); reporting to National Institute of Public Health; cooperation in national and international projects. Information from the questionnaires helps us to improve and upgrade our services and programmes and to cooperate in development of national HIV/STI strategies.
Data transmission
To verify linkage to care in case of a reactive HIV or Gonorrhea test result, your date of birth will be transmitted to the Institute of Microbiology and Immunology (an institution which is part of the Faculty of Medicine of Ljubljana University and performs laboratory analysis of biological samples collected at our testing). According to contracts, national legislation and EU regulations, we can transmit data to authorized persons and competent authorities, who are authorized by law to collect and process data.
Storage of data
Your data is stored for the duration of the prescribed time period or until the achievement of the purpose for which they were collected. When you are notified of the test results and the electronic entry with your code in the digital program has been processed, your phone number is automatically deleted. Data from this questionnaire is stored in electronic version on German server Contabo GmbH. The data collected during the previous year is destroyed on the last working day of January. Until it is destroyed, the data is kept in locked cabinets and in password protected files.

By filling out this questionnaire and giving it to the counsellor, you consent to the use of information from the questionnaire in aggregated form for research and statistical purposes. If you do not agree with these terms and conditions, you can withdraw from testing at any time.
Consent to the collection of personal data
I agree that DIC Legebitra collects my personal data (phone number) for the purpose of communicating the test results. The data is used exclusively for the purpose of communicating test results and is in no form passed on to other persons. Terms and method of storage: The data is stored in accordance with the prescribed retention periods or until the purpose for which it was obtained is fulfilled. The telephone number is destroyed immediately after the call or no later than 15 days from the date of obtaining the data. The telephone number is stored in electronic form in a separate database, which is not linked with your questionnaire, your unique code or other personal data. Only test consultants have access to your phone number, and the information is protected by two-step verification.
